Women leaving the doctor’s office with osteoporosis prescriptions usually walk out holding a pill bottle and a bottle of calcium. It feels routine. Safe, even. For decades, this has been the gold-standard protocol. No one questions it because it should make sense.
But new data suggests the calcium supplement might be doing less work than we thought. Meanwhile, a different nutrient is quietly pulling the weight for bone health. If you are on antiresorptive drugs, you might be overlooking the real lever that determines if your medication actually works.

Researchers at the Geneva Retirees Cohort didn’t just guess; they tracked 586 older women over a median of 3.5-years. The team used imaging to measure hip bone density, strength, and structure. They also used a validated food questionnaire to track calcium and protein intake at multiple intervals.
Here is the twist that most patients miss: 71% of the women in the study were already hitting their daily calcium recommendations through food alone.
Calcium Supplements Didn’t Move The Needle
For the vast majority of these women, popping extra calcium pills changed nothing.
Neither total calcium intake nor the act of supplementing itself significantly shifted how their bones responded to the drug. The data is blunt: if you are getting enough calcium from diet, adding more doesn’t help the medication work better.
There is a floor, not a ceiling.
The researchers found one critical exception. Women with very low dietary calcium intake who took no supplement saw no measurable benefit from the medication at all. So calcium is still mandatory. But for most people meeting that baseline, the supplement is just expensive urine.
And at high doses? It can bring digestive discomfort, kidney stones, or even cardiovascular concerns depending on the research.
Low Protein Blunted Drug Effects
While calcium was playing it safe, protein was showing up for the fight. Or not.
Women eating below the minimum recommended amount of protein daily saw significantly weaker results. It wasn’t a subtle difference.
Their hip strength declined at a rate of 2.6% per year.
Compare that to just 0.5% decline in women eating enough protein.
The gap was even wider in the trabecular bone—the inner, more porous layer of the hip where fractures actually happen. Low-protein women lost this structure at 2.8% annually. Women meeting the protein threshold? Just 0.3%.
Crucially, the researchers adjusted for calcium intake. The protein gap remained. This proves the effect is independent. Women who eat more protein don’t just eat more calcium; protein itself is doing something distinct for bone integrity.
Why Protein Is The Missing Link In Osteoporosis Management
Protein doesn’t just build muscle. It signals the body to maintain bone.
It triggers IGF-1 (insulin-like growth factor 1). This growth factor tells the body to build and preserve bone tissue. Protein also enhances how well your intestines absorb calcium and preserves muscle mass. Since muscles place load on bones during movement, stronger muscles mean denser bones.
When protein is low, these signals weaken. The medication tries to work, but without the raw materials and the biological signals provided by adequate protein, it can’t perform at its best.
How To Optimize Your Osteoporosis Medication With Diet
Two nutrients drive this process. Most women are obsessing over one while neglecting the other.
Calcium is still needed, but get it from food first. Dairy, canned salmon with bones (yes, bones), fortified plant milks, almonds, or dark leafy greens should be the foundation. Only add a modest supplement if you consistently fall short through diet.
Protein, however, needs to go up.
Many experts in bone health now suggest older women aim higher than standard minimums, especially during and after the menopause transition. Typical diets often fall short without anyone realizing it.
